 - Added app.config file for storing opt-in setting
 - Added CustomerFeedbackForm for opt-in
 - Added link to open customer feedback form to the main form
 - Added two methods to MainFormAudioGraph.cs that are called when effects are added and removed. These methods take the effect name as an argument. Dotfuscator injects code into these methods that sends a Feature Tick with the effect name as an extended key.
 - Added code to Program.cs that reads the opt-in preference when starting. Added methods for injecting RI Setup and teardown code.
